Introduction to SmartSheets™

Getting Started:

InterQual® SmartSheets™ for the Procedures and Imaging Criteria offer providers a user-friendly decision support tool at the point-of-care. The criteria in the form of SmartSheets™ make completing and sending requests fast and efficient.

Each InterQual® SmartSheet™ contains criteria in a checklist format and can be printed from the web page. To access a SmartSheet™ simply login to mynhp.com and click on the SmartSheets™ link on the left menu. Search from the SmartSheet™ menu, select the sheet of your choice and print. When you print a SmartSheet™ , the NHP Pre-certification Form can also be printed at this point. Click here to view examples of correctly completed SmartSheets™.

Completing the Forms:

Please complete the member information on the top section of the SmartSheet™ and proceed to check off the appropriate boxes that correspond with the patient information. Please note that a Pre-Certification Form also needs to be completed.

Faxing the Completed Forms:

The SmartSheet™ and the Pre-certification form can be faxed to Medical Management at (305) 715-2468 or (800) 305-0651, for outside Miami-Dade County.

Access to SmartSheets™

Gaining access to the InterQual® SmartSheets™ is easy!

1. Login with your myNHP.com username and password. (Click here to Login or Register now) Hint: Bookmark the login page so you can easily access SmartSheets™ on your next visit.


2. Click on the SmartSheets™ link located on the left navigation menu.


3. Select the SmartSheet™ of your choice.
